What is Greater Trochanteric Pain Syndrome?

Greater trochanteric pain syndrome (GTPS) is a common condition causing pain on the outer side of the hip. The greater trochanter is the bony prominence you can feel on the outside of your hip where several important muscles and tendons attach.

Previously called trochanteric bursitis, we now understand GTPS often involves tendon problems (gluteal tendinopathy) rather than just bursa inflammation. The condition is particularly common in middle-aged women and runners.

Symptoms

  • Pain on the outer side of the hip
  • Tenderness when pressing on the outer hip
  • Pain that worsens when lying on the affected side
  • Pain with climbing stairs or getting up from sitting
  • Pain that radiates down the outer thigh
  • Stiffness in the hip, especially in the morning

Risk Factors

  • Middle-aged women (most common demographic)
  • Runners and athletes
  • People with leg length discrepancy
  • History of hip surgery
  • Low back or knee problems affecting gait

What Causes GTPS?

Gluteal Tendinopathy

Degeneration of the gluteus medius and minimus tendons

Trochanteric Bursitis

Inflammation of the bursa over the greater trochanter

IT Band Syndrome

Tightness of the iliotibial band rubbing over the trochanter

Hip Abductor Weakness

Weakness in the muscles that move the leg outward
